Sounds like your resolution is degrading. You could try adjusting via the Windows Control Panel; I suspect a new monitor may be in order - though 4 years is definately NOT the norm for this to happen!

for some strange reason it has been perfect the last couple of nights, however just switched it on and everything is 'blurred' with a shadow again

would it be the screen / monitor or computer itself

It may be a problem with the graphics card. Does your computer have an add on card or is the graphics onboard?
